NEW & TRANSFER STUDENT REGISTRATION
Eastwood SDA Junior Academy is currently registering students for the 2024-2025 school year. Students are accepted on an individual basis. Please follow the steps below.
I. Submit all applicable Registration Forms

Parent/Guardian Pledges

Medical Consent to Treat
-
Physician’s Report (Required for K, 4th, and 7th)

Student Information Form for each Student

Family Information Form
-
Copy of Student’s Birth Certificate
-
Record Release for Previous School (Transfer Student)

Teacher Recommendation Form (Transfer Student)
II. Obtain Financial Clearance

Pay the Non-Refundable Registration Fee

Submit a Tuition Information Form

Submit a FACTS Tuition Management Form
III. Schedule the Following:

Entrance Interview with the Principal

Readiness Test for Kindergartners

Math Placement Test for Grades 1-8
RETURNING STUDENT REGISTRATION
Returning students must complete the registration process before being admitted.
